How Is Trauma Connected to Addiction?
Most people who come to Holina Rehab for addiction treatment are also carrying unresolved trauma. Trauma is rarely a stand-alone diagnosis — it is often the hidden driver beneath compulsive substance use, and addiction frequently develops as a way of managing the symptoms trauma leaves behind.
Unresolved trauma affects the nervous system in one of two directions. Hyperarousal keeps a person in a constant state of threat — anxiety, irritability, panic, and a readiness to fight. Hypoactivation is the opposite: a shutdown response of withdrawal, numbing, and hopelessness. Many people use alcohol or drugs to manage whichever state they are stuck in — substances to quiet hyperarousal, or to escape numbness — which is one of the clearest links between trauma and addictive behaviour.
Because trauma presents differently in every person, our trauma treatment is not a single therapy but an integrated set of modalities — including EMDR, somatic therapy, trauma-informed CBT and DBT — selected to match how each client’s trauma manifests, and delivered as part of the same clinical plan as their addiction treatment, not as a separate track.
Over 4 in 10 U.S. adults with PTSD (about 45%) also have problems with drug or alcohol use (National Center for PTSD, U.S. Dept. of Veterans Affairs, updated 2025). Among people entering treatment for substance use, research finds high rates of co-occurring PTSD among this population — with some studies placing the rate as high as 20–50% (Van den Brink, European Journal of Psychotraumatology, 2015). Because of this overlap, the American Psychological Association’s clinical practice guideline for PTSD recommends trauma-focused therapies — including trauma-focused CBT and EMDR — as effective treatment (APA Clinical Practice Guideline for PTSD, 2017).
