What Is NARM Therapy and How Does It Address Attachment Trauma in Addiction?


NARM — the NeuroAffective Relational Model, developed by Dr. Laurence Heller — is a therapeutic approach for attachment, relational, and developmental trauma, sometimes referred to clinically as Complex Trauma (C-PTSD). Rather than asking clients to repeatedly revisit and narrate painful memories, NARM works with what is happening in the present moment, tracking how the nervous system responds to connection, disconnection, and perceived threat.

For many people in addiction treatment, these early attachment wounds — neglect, emotional abandonment, or inconsistent caregiving — are not a separate issue from the addiction; they are frequently what the addictive behaviour has been organised around. Substance use can become a way to regulate a nervous system that never learned safety, or to manage the shame and disconnection that unresolved attachment trauma leaves behind.

Within Holina Rehab’s Dual Treatment programme, NARM is used alongside CBT, DBT, and EMDR to treat the substance use disorder and the attachment trauma underneath it at the same time, integrated with body-based and somatic work. Treating the addiction without addressing what the nervous system has been protecting against leaves the underlying conditions for relapse in place.

Clinical Fact — Developmental Trauma & Addiction

An estimated 75% of women and men in substance use treatment report a history of abuse or trauma (SAMHSA, Trauma-Informed Care in Behavioral Health Services, TIP 57). The original CDC-Kaiser Adverse Childhood Experiences (ACE) Study found that people reporting five or more adverse childhood experiences — including neglect, abuse, and household dysfunction — were seven to ten times more likely to report illicit drug use than those with none (Dube et al., 2003, Pediatrics).
