What Is Aquatic Tuning?


Aquatic tuning is a body-based somatic therapy that uses the physical properties of water — buoyancy, warmth, gentle resistance — to help clients access and release trauma that talk therapy alone can struggle to reach. At Holina Rehab, we use it because addiction so rarely exists on its own: for many residents, substance use developed as a way of managing a nervous system that trauma left stuck in a fight, flight, or freeze state.

Guided one-to-one by a trained therapist, sessions use the calming effects of water to lower physiological arousal, increase body awareness, and support the kind of emotional release that can be difficult to access sitting in a chair. The buoyancy of water reduces physical tension and allows the body to settle, creating the conditions for an overactive nervous system to come down out of chronic stress.

Aquatic tuning is not delivered as a standalone retreat activity. It is one part of a resident’s individual treatment plan, used alongside CBT, DBT, and the wider clinical programme, under Holina’s Dual Treatment model for addiction and the trauma underneath it.

Clinical Fact — Body-Based Trauma Therapy

A 2017 randomized controlled trial of Somatic Experiencing — a body-based trauma therapy built on the same nervous-system principles as aquatic tuning — found significant reductions in PTSD symptom severity (Cohen’s d = 0.94–1.26) and depression (d = 0.7–1.08) compared with a waitlist control group (Brom et al., 2017, Journal of Traumatic Stress). Trauma and substance use disorders also frequently co-occur, and addressing both together — rather than treating addiction alone — is associated with better outcomes (Van den Brink, 2015, European Journal of Psychotraumatology). One-to-One, Clinically Integrated


Each aquatic tuning session is personalised and guided one-to-one by a trained therapist, in a safe, supervised setting. Whether a resident is working through PTSD, emotional trauma, or physical stress held in the body, sessions are paced to what that person can process on that day.

Sessions are scheduled as one part of a resident’s wider treatment plan — delivered alongside CBT, DBT, and group therapy, and combined where appropriate with other holistic therapies such as yoga, mindfulness, and somatic work — never as a stand-alone activity separate from clinical care.
