Anxiety Treatment at Holina Rehab

Anxiety and addiction are deeply connected — for many, the two rarely exist in isolation. This page explains how anxiety and addiction reinforce each other, the types of anxiety we address, how anxiety is treated within our Dual Treatment programme, why a residential setting on Koh Phangan supports recovery, and how to reach our clinical team.

Anxiety and Addiction — A Two-Way Relationship

Anxiety and addiction rarely exist in isolation. For some, anxiety was present before the addiction — and the substance became the most effective available relief. Alcohol for social anxiety. Benzodiazepines for panic. Cannabis for generalised worry. The relief was real, even if the solution became the problem. For others, addiction created anxiety they had never experienced — the neurological disruption of sustained substance use leaving the brain in a state of persistent threat.

Treating one without the other leaves the person vulnerable to returning to what worked. Holina addresses anxiety as part of a fully integrated Dual Treatment programme — psychological support and counselling delivered alongside addiction treatment, not as a separate or optional add-on. No siloed pathways: the clinical team works across both simultaneously.

Clinical Fact — Anxiety & Addiction

Anxiety and substance use disorders co-occur at clinically significant rates: in a large U.S. epidemiological study (NESARC), 17.7% of people with a substance use disorder had at least one independent anxiety disorder in the same 12-month period. More broadly, 35% of U.S. adults with another mental disorder also have a co-occurring substance use disorder (NIDA, 2023 NSDUH). Peer-reviewed research further links untreated co-occurring anxiety to a heightened and earlier risk of relapse after addiction treatment. Holina’s Dual Treatment programme addresses anxiety and addiction together, for this reason.

Recognising the Problem

Types of Anxiety Holina Treats

Anxiety shows up in different forms, and often connects to addiction in different ways. These are the presentations our clinical team most commonly works with.

Generalised Anxiety Disorder

Persistent, pervasive worry that is difficult to control and affects daily functioning.

Social Anxiety

Fear of judgement or scrutiny in social situations, often self-medicated with substances.

Panic Disorder

Recurrent panic attacks — sudden, intense episodes of fear with physical symptoms.

Health Anxiety

Hypervigilance around bodily sensations and health, often heightened during withdrawal.

Trauma & PTSD-Driven Anxiety

Anxiety rooted in past trauma, frequently a primary driver of addictive behaviour.

Performance Anxiety & Perfectionism

Chronic pressure and fear of failure that can drive substance use as a coping tool.

Anxiety That Has Driven Addiction

Cases where anxiety preceded the addiction and the substance became a form of relief.

Post-Addiction Anxiety

Anxiety surfacing in early recovery as the brain and nervous system re-regulate.

Our Programme

How Is Anxiety Treated at Holina Rehab?

Anxiety is treated at Holina Rehab through a four-stage residential pathway — assessment, stabilisation, active treatment, and aftercare — delivered as part of our Dual Treatment programme alongside addiction care, never as a stand-alone service.

1

Assessment

Comprehensive assessment of anxiety presentation, history, and how it connects to the addictive behaviour — before the programme begins.

2

Stabilisation

Physical stabilisation and environmental settling. For many, anxiety reduces significantly in the first week through structure, safety, and removal from triggers.

3

Active Treatment

CBT, DBT, mindfulness, EMDR, somatic work, and group therapy working in parallel — addressing anxiety and addiction simultaneously.

4

Aftercare

Discharge planning includes ongoing anxiety management support, therapeutic referrals, and the relapse-prevention work that protects both recovery and wellbeing.

How Holina Treats Anxiety

  • CBT

Identifying and restructuring the thought patterns that maintain anxiety.

  • DBT

Distress tolerance and emotional regulation skills for moments of acute anxiety.

  • Mindfulness-Based Stress Reduction

Structured mindfulness practice to reduce reactivity and rumination.

  • EMDR

Used where trauma underpins the anxiety, to process the underlying material directly.

  • Breathwork & Somatic Regulation

Body-based techniques that calm the nervous system in real time.

  • Group Therapy

Normalising anxiety within a peer community, reducing isolation and shame.

  • Sleep Hygiene & Lifestyle Foundations

Addressed directly as core, not incidental, elements of anxiety management.

  • Medication Review

Reviewed by our clinical team where appropriate, alongside — never instead of — therapeutic work.

What the Programme Provides

Why Residential Treatment Works for Anxiety

Removed from Triggers

Residential treatment in Koh Phangan creates distance from the environments, people, and situations that maintain anxiety — giving the nervous system room to regulate.

Structured Days

Predictable daily structure reduces the ambient uncertainty that fuels anxiety. Routine is a clinical tool, not just an operational necessity.

Nature & Movement

Koh Phangan’s natural environment, yoga, and physical activity are integrated into the programme — each with demonstrable effects on anxiety regulation.

Community Support

Shared experience with peers in recovery normalises anxiety and reduces the isolation and shame that amplifies it.

Sleep & Nutrition

Disrupted sleep and poor nutrition are primary drivers of anxiety. Both are addressed directly as part of the residential programme.

Aftercare Planning

Anxiety management does not end at discharge. Aftercare planning identifies ongoing therapeutic support and the skills needed to maintain progress.

Common Questions

Anxiety Treatment at Holina: Frequently Asked Questions

Can I come to Holina for anxiety alone, without an addiction?

No. Holina is a licensed addiction treatment centre, not a psychiatric or stand-alone behavioural health facility. We treat anxiety as part of our Dual Treatment programme — always alongside addiction treatment, never as a separate service someone could enrol in on its own. If anxiety is your primary concern without a co-occurring substance use issue, a psychiatric or behavioural health specialist would be the appropriate first point of contact.

How does Holina treat anxiety alongside addiction?

Through our Dual Treatment programme, the clinical team addresses anxiety and addiction at the same time rather than treating one and referring the other elsewhere. This includes CBT, DBT, mindfulness-based approaches, EMDR where trauma is present, breathwork and somatic regulation, and group therapy — delivered within the structure of residential care.

Will my anxiety get worse before it gets better in early recovery?

It’s common for anxiety to surface or intensify in the first days of stopping substance use, as the brain and nervous system re-regulate. Our clinical team monitors this closely during the assessment and stabilisation stages, and for many residents anxiety noticeably eases within the first week once structure, safety, and clinical support are in place.

What therapies are used for anxiety at Holina?

Our approach combines evidence-based talk therapies (CBT and DBT), trauma-focused work such as EMDR where relevant, mindfulness-based stress reduction, breathwork and somatic regulation, group therapy, and attention to sleep and lifestyle foundations — all integrated with addiction treatment rather than run as separate tracks.

How long does treatment take?

Most residents follow a 28, 60, or 90-day residential programme. The right length depends on the severity of both the anxiety and the addiction, and is set during your initial assessment. Learn more about our assessment and detox process.

Does insurance cover treatment for anxiety and addiction?

Holina Rehab is a private-pay facility and does not bill insurance directly. Residents can use our financing options to fund treatment. Coverage depends on your provider and policy. See rehab insurance and financing for options, or how much rehab costs in Thailand, and contact our team to check your specific cover.

Anxiety and Addiction, Treated Together

Our admissions team can talk through your specific situation — what the anxiety looks like, how it connects to the addiction, and how Holina addresses both, seven days a week, in full confidence.
